The LBR network settings article says that for Direct Routing, any PSTN call to or from an LBR-enabled user whose external IP matches no trusted IP (an unknown location) is blocked, without mentioning the GatewayLbrEnabledUserOverride exception documented in the planning article and cmdlet reference.
